DA Form 2404, Equipment Inspection and Maintenance Worksheet, is the working document a soldier or maintainer uses to record the deficiencies and shortcomings found while performing a preventive maintenance checks and services (PMCS) inspection on a piece of equipment. Its use is prescribed by Department of the Army Pamphlet 750-1, Army Maintenance Management, and it is a manual counterpart to DA Form 5988-E, an electronically generated worksheet produced by systems such as the Global Combat Support System-Army; the two are not interchangeable line for line, but they record the same categories of fault data.

The inspector lists each fault by item, records whether it renders the equipment non-mission-capable, and signs the worksheet, which the unit's maintenance section then uses to open work orders and, ultimately, to update the equipment's permanent historical maintenance record. Because the 2404 is the point of first entry for nearly every maintenance fault the Army tracks, its accuracy directly affects unit readiness reporting. The current edition is dated August 2026, replacing an edition that had stood since February 2011.
