DA Form 3161, Request for Issue or Turn-In, records the transfer of property between a property book officer, a hand receipt holder, and a sub-hand receipt holder — the routine issue and turn-in transactions that keep an Army unit's supply accounts current between formal inventories. Its use is prescribed by Army Regulation 710-4, Inventory Management: Property Accountability, the same consolidated regulation that governs DA Form 2062. Where a shipment needs more line items than the base form provides, the continuation sheet DA Form 3161-1, dated August 2011, extends it.

A requesting activity lists the items, quantities, and authorization documents needed on the form and routes it for approval before the issuing activity releases the property; a turn-in follows the same document to record property being returned to accountable stock. Local supply procedures may substitute DD Form 1150 or DD Form 1348-1A in place of a DA 3161 as the change document when circumstances call for it, but the 3161 remains the standard Army form for this transaction at the organizational level. The current edition, dated December 2023, replaced an edition dated December 2017.
