DA 3645 — Organizational Clothing and Individual Equipment Record

DA Form 3645, Organizational Clothing and Individual Equipment Record, tracks the Organizational Clothing and Individual Equipment (OCIE) — field gear such as body armor, cold-weather items, and load-bearing equipment — issued to a specific soldier, as distinct from the general property recorded on a DA Form 2062. Its use is prescribed by Army Regulation 710-4, Inventory Management: Property Accountability. Where a soldier's OCIE list runs longer than the base form's line items, the continuation sheet DA Form 3645-1 extends it.

Central Issue Facility (CIF) personnel initiate and maintain the record as items are issued to and turned in by an individual soldier over the course of a career, updating it at reassignment, promotion, or whenever equipment is replaced or exchanged. Because OCIE follows the soldier rather than staying with a unit's property book, the 3645 is the accountability document a CIF or gaining unit consults to verify what a soldier already holds before issuing duplicate items, and it is what a soldier clears against at separation, retirement, or permanent change of station out-processing. The current edition, dated December 2023, replaced an edition dated October 1991.
