The continuation sheet for DD Form 1149, used when a manual requisition or shipment has more line items or shipping containers than the base form allows.

DD Form 1149C, Requisition and Invoice/Shipping Document (Continuation Sheet), extends DD Form 1149 when a manual requisition, invoice, or shipment has more line items, or more shipping containers to tally, than fit on the base form. Like DD 1149 itself, its completion is documented within DoD 4161.2-M, the DoD property disposal and accountability manual that historically governed the 1149 family of forms, and its Office of Management and Budget information-collection control number, 0704-0246, is the same one assigned to DD Form 1149.

The shipping activity attaches a 1149C to the base DD 1149 whenever the transaction's line-item data, or the shipping-container tally recording how many individual containers make up the shipment, runs past what the single-page 1149 provides space for. Because the 1149C only ever travels attached to a base DD 1149 — it is not used as a stand-alone document — it sees exactly the same use cases as its parent form: lateral property transfers and Government-Furnished Property shipments to contractors outside MILSTRIP channels, just at a larger scale than the base form alone can record. The current edition is dated January 2016, matching the DD 1149 edition it accompanies.
