DD Form 1155, Order for Supplies or Services, is the Department of Defense's general-purpose order and delivery form for placing orders under simplified acquisition procedures, against a Blanket Purchase Agreement (BPA), or against an existing indefinite-delivery contract. Its completion is prescribed in the Defense Federal Acquisition Regulation Supplement's Procedures, Guidance, and Information (DFARS PGI 253.213-70 and 253.2).

A contracting officer issues the form to place the order, and — because its fields mirror those of DD Form 250 — the same document can double as the receiving report once the supplies or services are delivered and accepted, reducing the paperwork needed to close out a small or recurring order. It sees the heaviest use where speed matters more than a full solicitation: fast-turnaround simplified acquisitions, task orders released against standing BPAs, and orders placed under a schedule contract. Because it functions as both the ordering instrument and, potentially, the acceptance record, accuracy in the delivery schedule, funding citation, and inspection/acceptance blocks on the 1155 has direct downstream effects on invoicing and payment.
