DD 1348-1A — Issue Release/Receipt Document

DD Form 1348-1A, Issue Release/Receipt Document, is the core shipping and receipt document of the Military Standard Requisitioning and Issue Procedures (MILSTRIP) system, governed by the Defense Logistics Agency's Defense Logistics Management Standards (DLM 4000.25, Volume 2). A DLA distribution center, military supply activity, or contractor issuing materiel against a MILSTRIP requisition completes a DD 1348-1A to serve simultaneously as the shipping document, packing list, and receipt copy that travels with the shipment.

The form's fixed-field layout is also the data source for the standard DoD shipment-unit barcode label prescribed by MIL-STD-129, which is why the 1348-1A format underlies both the paper document and the machine-readable label affixed to the shipment. The receiving activity signs and returns a copy — or confirms receipt electronically — to close out the transaction against the original requisition. Because it is the transaction record for nearly every depot-level materiel release in the DoD supply chain, the 1348-1A is one of the most frequently referenced forms in logistics, transportation, and receiving operations.
