DD Form 1348-5, Notice of Availability/Shipment, notifies a requisitioner or consignee that materiel they requisitioned is available for release or has already shipped. It is documented within the Defense Logistics Agency's Defense Logistics Management Standards manual, DLM 4000.25, alongside the related shipping and receipt documents DD Form 1348-1A and DD Form 1348-6. The Notice of Availability process the form supports gives the International Logistics Control Office visibility into Foreign Military Sales shipments in particular, though the form is not exclusive to FMS transactions.

A supply source completes the 1348-5 to identify the requisition being satisfied, the item and quantity involved, and either its availability for pickup or its actual shipment details, and routes it to the requisitioning activity or consignee named on the original requisition. In the automated MILSTRIP environment, the same data is carried electronically as a DLMS 856N Notice of Availability transaction, but the DD 1348-5 layout remains the paper reference format and is still used where a manual or non-automated notice is required. The current edition is dated January 2015; older mirrored copies bearing a June 1969 edition stamp circulate on third-party sites and should not be relied on for current use.
