DD Form 1348-6, DoD Single Line Item Requisition System Document (Manual — Long Form), is the manual continuation form used with DD Form 1348 when a requisition for a National Stock Number or part-number item needs more descriptive data than the base form's fields allow. It is documented within the Defense Logistics Agency's Defense Logistics Management Standards manual, DLM 4000.25, Volume 2, alongside the short-form DD 1348-5 and the related DD 1348-8.
Activities without access to an automated MILSTRIP requisitioning terminal — a deployed unit, a small activity, or an emergency situation — prepare the 1348-6 by hand or typewriter to requisition materiel through the standard supply system when the identifying data (nomenclature, specifications, part-number cross-references) will not fit on the shorter DD 1348 card format. Because it is a manual fallback rather than the everyday transaction record, it is used less often than the 1348-1A but remains part of the standard MILSTRIP document set that every supply and logistics activity is expected to recognize and be able to complete. The current edition is dated February 1985, superseding an April 1977 edition.
