DD 361 — Transportation Discrepancy Report (TDR)

DD Form 361, Transportation Discrepancy Report (TDR), documents loss, damage, or another discrepancy discovered in DoD cargo during transportation — as distinct from a Supply Discrepancy Report, which covers packaging or quantity problems found when a shipment is received against a requisition. Its use is prescribed in the Defense Transportation Regulation, DTR 4500.9-R, Part II, issued by U.S. Transportation Command.

The shipping or receiving activity that discovers the discrepancy completes the form to describe the shipment, the carrier and mode of transportation involved, and the nature of the loss or damage, then routes it to the responsible transportation office for investigation, corrective action, and, where warranted, a loss-and-damage claim against the carrier. Because the TDR is the record that starts a carrier liability claim, activities are expected to file it promptly and retain supporting documentation — bills of lading, delivery receipts, photographs — alongside it. DD Form 361 replaced the earlier Standard Form 361, which had a March 1984 edition, and a continuation version, DD Form 361C, is available for shipments with more discrepancy detail than the base form's fields accommodate. The current edition is dated June 2006.
