SF 364 — Report of Discrepancy (ROD)

Standard Form 364, Report of Discrepancy (ROD), is the government-wide form for reporting a shipping or packaging discrepancy — the wrong item, an incorrect quantity, or inadequate packaging — that is the responsibility of the shipper rather than the receiving activity. Its use was prescribed under the Federal Property Management Regulations, FPMR 101-26.8, and the form is maintained by the General Services Administration.

A receiving activity that finds a discrepancy at the point of receipt — as opposed to damage discovered in transit, which is reported on a Transportation Discrepancy Report instead — completes the SF 364 to describe the shipment, the discrepancy, and the corrective action requested, and routes it to the shipping source for resolution. Within the Department of Defense, discrepancy reporting of this kind is more commonly handled today through the DLMS Supply Discrepancy Report (SDR) process built on DD Form 1348-1A shipment data, but SF 364 remains the standing government-wide form referenced in property management regulation for reporting a discrepancy against a civilian or interagency shipment. The current edition is dated February 1980 and has not been revised since.
