DD FORM 2872 System Authorization Access Request SAAR

The System Authorization Access Request (SAAR) is DD Form 2875 — the standard DoD form used to request, change, or terminate a user's access to a Department of Defense information system. The directory title "DD Form 2872" is a form-number error: DD Form 2872 is a real but unrelated DoD form (the Depleted Uranium (DU) Questionnaire used in post-deployment health assessments), not the SAAR. What it is

DD Form 2875 records a user's identity, the access being requested, and the justification for it, then routes the form for supervisor endorsement, verification of need-to-know, and information-system-owner/ISSO authorization. It has four parts: Part I (user information and signature), Part II (endorsement and need-to-know verification), Part III (certification of background investigation or clearance), and Part IV (system-specific, customizable by the hosting DoD component or system owner). Access

Completing and submitting the form, by contrast, is how a user requests access to a specific gated DoD system — the completed form is routed through that system's own approval chain, not through WHS.
