PRE-NEED ELIGIBILITY FOR INTERMENT IN A NATIONAL CEMETERY

What it is

Applicants apply online at VA.gov, or by submitting VA Form 40-10007 by mail to the NCA Evidence Intake Center (PO Box 5237, Janesville, WI 53547) or by fax to the National Cemetery Scheduling Office. The resulting pre-need decision letter does not reserve a specific cemetery or gravesite — those cannot be reserved in advance — and eligibility may be re-reviewed at the time of actual need if laws or circumstances have changed.

Access

The program is public — any veteran or eligible family member can apply without a CAC or special account, either online through VA.gov or by mailing/faxing VA Form 40-10007. Applicants generally need Social Security number, date of birth, military service history, preferred cemetery (for records purposes only), and a copy of DD Form 214 or other separation documents if available. Accredited veterans service representatives can assist with the application.
